MSDP will need a completed Intake Form for us to be able to care for your Service Dog at no cost to the handler.

At this time we are only able to take care of Service Dogs.
We understand that there is a need for pet sitting care to be provided for emotional support dogs and dogs that are pets as well, so we are currently working on ways to expand our services and hope to be able to do so sometime in 2025.
MSDP will need a completed Extension Form for us to be able to extend the time that we care for your Service Dog.

It is VERY important that an Extension Form be completed by the service dog handler or case worker/contact person listed on the intake form IF the handler is getting additional medical care and/or staying in a program longer than the original "end date" that was written on the intake form.

Failure to do so could result in the service dog being re-homed.
